Clerk notices & record demands in QPSG
A notice to a clerk or a demand to correct a record is often copied from an old form: please be advised, the undersigned hereby, will file, and a date with no seal. The clerk still has to read a name, a fact, and a request. This article is about that language — not about what any court must accept.

What usually goes wrong
- Courtesy fluff — “please be advised,” “kindly take notice” — no named party and no present fact.
- Future tense — “will file,” “shall appear,” “may be deemed” — the notice talks about a later event instead of the present claim.
- Unnamed actor — “the undersigned,” “this office” — the record does not say who is noticing whom.
- Unsealed date — “January 15, 2025” with no tilde seal, so the date reads as ordinary prose.

PLEASE BE ADVISED that the undersigned hereby gives notice that he will file a demand for correction of the record with the Clerk, and that the Clerk shall enter the same as soon as practicable. Dated this 15th day of January, 2025.
